French Trains too Wide for Stations – Student Presentation

At first glance, it’s an engineering error. French public railways operator ordered new trains, only to discover that they were too wide for the platforms in many stations. But, as analyzed by a group of Masters Students’ of the Error Management class at Solvay Brussels School of Economics & Management, the errors were by and large organizational, not only technical. Worse still, executives had received many signals that they left unattended to.
